Thank you for your interest in Derivative Psychiatry, PLLC. Due to a recent increase in requests, we are temporarily pausing new patient request form submissions while we review those already received. 

We anticipate reopening the form in approximately one week. Please check back here for updated availability. 

If you have already submitted a request, there is no need to submit another. We will review submitted requests as availability permits and contact you regarding next steps. Submission of a request does not guarantee acceptance into the practice or establish a physician-patient relationship.
