New Patient Requests Are Paused Until Further Notice 

Thank you for your interest in Derivative Psychiatry, PLLC. The practice is currently at capacity, and we are pausing new patient request form submissions until further notice. 

We will update this page when we have additional availability and are able to accept new patient requests again. Please check back periodically for updates. 

If you have already submitted a request, there is no need to submit another. We will continue to review previously submitted requests as availability permits and will contact you regarding next steps. Submission of a request does not guarantee acceptance into the practice or establish a physician-patient relationship.
